Consent and Registration form

In joining this program you agree that your child maybe videotaped and be used for any promotional purpose. In consideration of the acceptance of my childs entry I do hereby for and on my behalf of myself and my heirs and legal representative release and forever discharge the Litchfield School District, St. Peter's Montessori  and A Little Drama and any instructors of any harm or injuries which may be suffered by me or my child in connection with A Little Drama. I covenant not to sue and agree to assume all risk associated with my participation in this program.*refunds will not be given after participation in class.
